Sponge coming out from a/c outlets inside the car?

Sponge pieces coming out from ac outlets inside the car whenever i switch the a/c. What does this mean? Do i need to change the air filter? Something in the a/c thats made of foam that is getting blown out of your vents by your blower fan.
